What is upper blepharoplasty?

Upper blepharoplasty is one of the facial aesthetic procedures most frequently performed by Dr. van der Veen. It involves removing or reshaping excess skin and fat from the upper eyelids and fat remodelling of the lower eyelids, to achieve a more youthful and natural appearance.

An individualized assessment is essential to determine whether excess eyelid skin is the sole cause of a tired-looking gaze. Sometimes, drooping eyebrows or a low position of the eyelid itself (eyelid Ptosis) also contribute to the appearance of heavy eyelids. In such cases, it may be advisable to combine upper blepharoplasty with a brow lift or eyelid elevation, correcting eyelid ptosis—as in the case of the patient shown below—to achieve a more harmonious result.

What does Upper Blepharoplasty surgery involve?

It is not simply a procedure to “remove skin and fat.” It is a precise surgery that evaluates various factors, such as eyebrow position, the eyelid crease, and volume loss in the eye area. In most cases, it involves a combination of skin removal, fat remodeling, and adjustment of the periocular muscles. Since each patient has unique characteristics, a detailed analysis is crucial in order to apply the most appropriate surgical technique.

The procedure is performed under local anesthesia, with or without sedation, ensuring patient comfort throughout the intervention. It does not require hospitalization, and recovery is relatively quick, with minimal side effects and barely visible scars hidden within the natural folds of the upper eyelid.

What can I do before surgery to optimize the result?

To achieve the best possible results, it is important to follow certain recommendations before surgery:

Discontinuation of anticoagulants: If you are taking blood-thinning medications, it is crucial to stop them several days before the procedure, always under medical supervision. The required washout period varies depending on the medication, so it is important to follow your doctor’s instructions.

Adequate rest: Avoid physical activity and sleep with your head slightly elevated during the first week, as this helps reduce postoperative swelling and the risk of significant bruising.

Quit smoking: It is recommended to stop smoking at least a few weeks before surgery. Smoking interferes with healing and increases the risk of complications.

Postoperative care

After upper blepharoplasty surgery, it is essential to follow certain guidelines to ensure a smooth and complication-free recovery:

Rest and limited activity: Avoid intense physical activity, lifting heavy objects, or performing household chores for at least 5–7 days. Physical exertion increases blood pressure and may cause bleeding or bruising that can negatively affect results.

Cold compresses: During the first 72 hours, applying cold compresses to the eyelids helps reduce swelling and bruising. It is recommended to do this for several minutes every hour.

Antibiotic ointment: Your surgeon will prescribe an antibiotic ointment that should be applied twice daily to the surgical wounds for the first 7 to 10 days.

Recovery and postoperative care

Recovery from an upper blepharoplasty is usually swift and varies slightly by the technique used. In general, patients may experience some swelling and bruising, but most can resume daily activities within a week.
