Lipofilling: what it is and how it works

Over time, the area around the eyes undergoes natural changes: the orbicularis muscle that protects the eye descends, the fat compartments of the cheeks diminish, and the bony rim of the orbit becomes more visible. These changes often lead to fat herniation in the lower eyelid, creating the well-known under-eye bags and a tired appearance.

Traditionally, these signs were treated with more invasive surgical techniques and visible incisions. Today, the approach is more subtle and respectful of anatomy: through an internal (transconjunctival) incision, orbital fat can be reshaped and repositioned to smoothen the transition between the eyelid and the cheek (see Dark Circles and Eyelid Bags).

However, in some patients this correction alone is not enough to achieve a satisfying rejuvenated appearance. In such cases, facial lipofilling (also known as autologous fat transfer or lipotransfer) is the best option to naturally restore lost volume. In our practice, facial lipofilling is most often performed together with lower blepharoplasty, as these two treatments complement each other perfectly.

Lipofilling for Dark Circles

Lipofilling for dark circles involves harvesting fat from the patient’s own body (usually from the thigh or abdomen) through a mini liposuction. After a purification process, the fat is carefully injected on the lower orbital rim, just beneath the eye. This smoothens the transition between the eyelid and the cheek, correcting hollowness and naturally improving a tired appearance.

Because the patient’s own tissue is used, it is biocompatible, long-lasting, and very natural. In addition, during the same procedure, the harvested fat can be used to treat other facial areas that have lost volume, such as the cheeks, temples, or brows.

When facial lipofilling is combined with procedures such as blepharoplasty or ptosis correction, the overall effect can be even more powerful.

Benefits of Facial Lipofilling

Facial lipofilling offers multiple advantages over other rejuvenation treatments, especially in the periocular area. Since it uses the patient’s own fat, it is very safe and provides volume restoration and natural-looking results. Key benefits include:

  • No risk of rejection: Does not cause adverse reactions such as encapsulation or allergies.
  • Versatility: Allows treatment of larger facial areas, as sufficient fat is usually available.

Possible drawbacks and limitations

Although facial lipofilling is a safe procedure with very satisfying results, it is important to be aware of its limitations:

  • Partial fat reabsorption: Some of the fat may be naturally reabsorbed in the following months, occasionally requiring a small touch-up session.
  • Initial swelling and bruising: Common during the first week, but usually resolve gradually and relatively fast.
  • Stabilization time: Final results are typically visible only after several weeks, once swelling subsides and the graft settles.
  • Irregularities in the treated area: Because fat is injected with a fine cannula, mild contour irregularities may occur.
  • Technique-dependent: Requires an experienced specialist, as precise fat placement is essential for natural, harmonious results.
  • Individual variability: The quality of the patient’s fat tissue and lifestyle influence graft survival.

Safety and longevity of results

Facial lipofilling is a safe procedure because it uses the patient’s own fat, eliminating the risk of allergic reactions or rejection. It remains a surgical manoeuvre however, and should only be performed by a specialised surgeon. As a fully biocompatible material, it almost never causes encapsulation or side effects related to synthetic substances.

Regarding longevity, part of the graft may be reabsorbed during the first months, but the fat that integrates successfully into the tissues remains stable long term. This means the results are natural and durable.

The Facial Lipofilling process

Before the procedure and preoperative tests

Before undergoing facial lipofilling, a complete evaluation of general health and the treatment area is performed. This includes reviewing medical history, current medications, and possible allergies, as well as basic blood tests. Your physician will explain the treatment goals, assess skin quality and the amount of fat available for transfer, and define the most appropriate technique for you.

Recovery and Postoperative care

After the procedure, it is normal to experience some swelling or mild bruising in the treated area. Most patients can return to their usual activities within a few days, avoiding strenuous physical activity and intense sun exposure during the first weeks. Naturally, recovery time is also dependent on other procedures performed simultaneously, if that is the case. 

Specific care measures are recommended, such as applying cold compresses and maintaining proper hygiene of the area, according to the surgeon’s instructions. Full integration of the injected fat occurs gradually, and final results are usually seen after several weeks.

Results of Lipofilling for Dark Circles (before and after)

The effects of facial lipofilling for dark circles are natural and harmonious. When dark circles are partly caused by skin pigmentation in the area, other non surgical treatment might be added or advised instead. By filling the lower orbital rim and smoothing the transition between the eyelid and the cheek:
