What is Blepharoplasty?
Blepharoplasty is one of the most frequently performed facial aesthetic procedures by Dr. van der Veen. It consists of removing excess skin, muscle, and, in some cases, fat from the upper eyelids to achieve a more youthful and natural appearance.
In the upper eyelid, excess skin is usually located between the natural eyelid crease and the eyebrow. Although in most cases the excess skin is in fact removed, the fat can also be reshaped or repositioned to achieve the best aesthetic and functional result. This is referred to as Volumetric Upper Eyelid Blepharoplasty.

Why is Blepharoplasty performed?
Blepharoplasty is primarily performed to correct excess skin, fat, or laxity of the eyelids that can give the face a tired or aged appearance or, in some cases, interfere with vision. Beyond the aesthetic benefit, this procedure can also improve the visual field when correction includes droopy upper eyelid (Ptosis) repair. A Blepharoplasty procedure restores a fresher, more rejuvenated look, boosting confidence and visual well-being.

Male Blepharoplasty
Surgery in men follows a similar approach to that in women, but with moderation in the removal of skin and fat, to maintain the masculina eyelid and eyebrow shape.
The male eyelid and eyebrow have a characteristically straighter shape that must be preserved to avoid feminizing the patient. The goal is to achieve a fresh, rested appearance without it being obvious that surgery has been performed.
Before and after Blepharoplasty
Preparation before blepharoplasty is the first step towards a successful procedure. By carefully following medical recommendations and arriving at surgery in the best possible condition, recovery is faster and results are optimised. The “before and after” of blepharoplasty typically reflects a brighter, younger, and more rested look, enhancing the natural facial features without losing authenticity. Preparing properly is an investment in the success of the procedure and in overall satisfaction of the cosmetic and functional enhancement.
How is the surgery performed and what type of anesthesia is used?
Before the procedure, an ophthalmologic and cosmetic evaluation is carried out, which may include tests to measure tear production when indicated and assess different eyelid and facial structures. A visual field examination can also be performed to detect possible alterations in peripheral vision when addressing droopy eyelid correction. In addition, photographs of the eyelids are taken from different angles, which helps with surgical planning and allows results to be compared after surgery.
Correction of the upper eyelid is usually performed under local anesthesia alone, but can also be performed under sedation.
The procedure begins by marking the natural eyelid crease and the amount of skin to be removed. Local anaesthetic is then administered and, once the area is numb, the excess skin is removed. If necessary, fat is removed or reshaped before closing the incision with sutures, which are removed at the clinic after about 7 days.
After surgery, wound dressings or an antibiotic ointment are applied to the incisions. You will need to apply this ointment along with cold compresses for several days to aid recovery.
Preparation for surgery: what can I do beforehand?
For optimal results, it is important to get adequate rest and maintain a balanced diet.
If you smoke, it is recommended to stop several weeks beforehand, as smoking impairs healing. In fact, many people use this surgery as motivation to quit smoking.
If you take anticoagulant medications, consult the doctor who prescribed them to evaluate the possibility of temporarily discontinuing them before surgery.
What to avoid before Blepharoplasty: key recommendations
Before blepharoplasty surgery, it is essential to avoid certain actions that could compromise safety and results. You should not take anticoagulants, anti-inflammatory drugs, or supplements that may increase bleeding unless your doctor authorises their use. Smoking and alcohol consumption are also not recommended in the weeks leading up to surgery, as both interfere with healing and increase the risk of complications. Avoid wearing makeup or applying creams to the eyelid area on the day of the procedure, and do not use contact lenses. Following these guidelines helps ensure optimal surgical conditions and reduces potential risks during and after the procedure.
The importance of being accompanied on the day of surgery
On the day of surgery, it is essential that the patient comes to the clinic accompanied by a trusted family member or friend. After the procedure, some drowsiness due to anaesthesia and a slight reduction in reflexes are common, so traveling alone is not recommended and operating a (motorised) vehicle is prohibited. Having a companion ensures a safe return home and provides support during the first postoperative hours, helping with any needs that may arise and contributing to a calmer, more comfortable recovery.
What to expect after the operation?
It is normal to experience swelling and tightness in the eyelids due to anaesthesia and postoperative inflammation as well as the presence of sutures. You may also notice temporary blurred vision caused by the ointment applied to prevent dryness, which resolves quickly.
You will have small sutures that will be removed after one week, and possibly some bruising that will gradually fade.
Postoperative care: what can I do afterward?

During recovery, avoid physical exertion, lifting heavy objects, sports, or intense household tasks for at least 5–7 days to prevent bruising or complications.
Applying cold compresses to the eyelids for 5-10 minutes every hour during the first 72 hours helps reduce swelling.
The prescribed antibiotic ointment should be applied twice daily for 7 to 10 days. In some cases, this ointment is also used inside the eye to prevent dryness, especially before sleeping and when droopy eyelid correction has been performed as well.
